In our experience these issues persist - and even where there is competent project management, a lack of exec support and sponsor / stakeholder engagement are often critical factors in projects not meeting expectations.

Another factor that often plagues projects is project managers who 'manage a plan' and not the project. ie. they are not keeping their eye on the risks and issues and don't take the time to dive into the content and context to be able to adequately manoeuvre the project in the right direction.
